The CBC Health Home provides comprehensive care coordination services to both adults and children on Medicaid throughout New York City.

CBC is a provider-owned organization and one of the largest Health Homes in New York State, serving over 17,000 enrolled members who have serious behavioral and/or medical disorders. These individuals receive assessment, service planning and care coordination. Our care management agencies (CMAs) support identification and seamless enrollment of eligible New Yorkers through outreach and engagement services.


Community-Based Organizations

CBC’s provider-led Health Home consists of nearly 50 community-based organizations which coordinate care for enrollees. Our CMAs draw on decades of experience providing care management for people with mental illness, medical conditions, substance use disorder and/or HIV/AIDS.

They employ many hundreds of care coordinators knowledgeable of community resources and services and speak a diverse number of languages. Their care coordination staff are trained in culturally competent and trauma-informed service delivery.


Citywide Service Network

CBC Health Home has a citywide service network that includes hundreds of hospitals, clinics and treatment programs, supportive housing providers, clubhouses, employment services and much more. The CBC network allows its Health Home to continue to provide comprehensive, quality services even as members move within treatment or housing settings across the five boroughs.

Understanding that when the right community services are in place, our members may avert crises that could result in a hospital visit, we partner with hospitals to ensure that Health Home members receive targeted assistance when they are discharged from an inpatient stay and after an emergency service visit.


Serving Children and Adults

CBC’s designation to serve children and adults allows for the continuity of care necessary for optimal planning as a young person matures, and new conditions and life events occur. CBC also serves adult and child family members in a family-centered approach, and can even assign a care manager to serve families with multiple members enrolled in our Health Home.


Modern Infrastructure

CBC’s Health Home infrastructure connects dozens of CMAs throughout the five boroughs in a citywide network. As a lead Health Home, CBC facilitates and supports effective care coordination services that improve our members’ health status and lower the cost of care by reducing avoidable hospitalizations and emergency service visits. Integral to CBC Health Home’s operations are:

  • A robust Health Information Technology (HIT) System;
  • Medicaid billing services;
  • Training opportunities;
  • Uniform clinical protocols, quality standards and quality improvement support; and
  • Full connectivity to Regional Health Information Organizations (RHIO).
